Special Application Form to discharge  imported Plant or Plant related product

An applicant has to fill in a prescribed application form and apply to the Director, Plant Protection Wing(Plant Quarantine) of DAE describing the following necessary particulars:

1.     Name & Quantity of the plants / plant product

2.     Name of the exporting country

3.     Country of Origin of the Product

4.     Expected date of arrival

5.     Point of entry & Purpose of import

6.     Name and detailed address of importer

7.     LC Number

8.     Bill of Lading/ Air Way Bill/ Challan

9.     Previously released product under Import Permit (IP)

10.  Ship Rotation Number

List to document to be submitted with Application Form

1.     Copy of Import Permit (IP)

2.     Copy of Phytosanitary Certificate

3.     Copy of Bill of Lading/ Air Way Bill/ Challan

4.     Certificate of Origin

5.     Copy of Invoice

6.     Copy of LC  issued by Bank
