Animal food Export/Import License Application Form

An applicant has to apply to the Director General, Department of Livestock Services in a prescribed application form describing the following necessary particulars:

1.     Name & Address of Proprietor

2.     Name & Address of Company

3.     Address of Warehouse

 List to document to be submitted with Application Form

1.     Attested copy of Export/Import License

2.     Original Money Receipt Copy (Application Fee)

3.     Income Tax Certificate

4.     Trade License
